Foothills Concussion & Rehabilitation Centre is a local non-profit organization dedicated to providing proven, specialized assessments and quality treatments to those living with Concussions and other neurological impairments including Attention Deficit Hyperactivity Disorder.

The effects of Concussions and other neurological impairments know no socioeconomic boundaries. As the fees for these treatments are not currently covered by our health-care system, many important Albertans including low-income children, members of our homeless population, and battered women, are not able to access the support and treatment they need.

If you are suffering from the effects of a Concussion or other neurological impairment, have a demonstrated economic need, and would like to receive subsidized treatment, we encourage you to download and fill out the attached PDF form.

Step 2: Submit the completed application form along with required documentation (listed in the form) by e-mail to fcrcentre@outlook.com or by mail or delivery to:

Step 3: The FCRC Board will review your completed application and supporting documentation to determine preliminary financial eligibility.

Step 4: If the FCRC Board determines that you are preliminarily eligible based on financial need, Dr. Stuart Donaldson will assess you in order to determine assess your neurological impairment.

Step 5: If Dr. Donaldson determines that you are eligible for treatment from a physiological perspective, the FCRC will require additional financial disclosure from you.

Step 6: Once you have submitted the additional financial disclosure required by the FCRC Board, the FCRC Board will make its final determination of eligibility for treatment subsidization.

Step 7: You will be notified of the FCRC's final determination by e-mail or telephone.

Please note: if you are unable to provide the required documentation due to socioeconomic circumstances, exceptions may be considered on a case-by-case basis.
